A Sample Form
for Conducting 360° Evaluation of Those Who Manage
People in Associations
This evaluation form is designed to
allow staff to evaluate the performance of their
supervisors, to provide feedback from which they can
learn and respond accordingly. The questions have been
phrased so that the grading scale remains consistent; if
the question contains the phrase does not, 5 (always)
would still be the most positive response.
All responses will be kept confidential.
Please feel free to make additional comments, either on
the form or on a separate sheet. When completed, please
fax to xxx, who will tabulate and report the results to
the Board. The Executive Director will only see
aggregate scores.
